What is whole person health?

Whole person health is a treatment approach that considers all factors that contribute to an individual’s health and wellness, addressing their physical, emotional, spiritual, social and environmental needs. It is patient-centered, empowering the individual to be the driving force in their own recovery.

The Behavioral Health Home (BHH) is an effective healthcare service delivery model to promote whole person health. It focuses on the integration of primary care, mental health, and social services to support adults and children diagnosed with mental illness. The BHH model of care uses a multidisciplinary team to coordinate and deliver person-centered services designed to support an individual in reaching their health and wellness goals.
